Please bring your driver’s license, insurance card and any medical records you feel would be helpful to this appointment. Arriving 15 minutes prior to your appointment will greatly facilitate the check-in process.

We realize that while BloomMed membership is AWESOME, it may not be the best fit for all of our amazing patients, so BloomMed now offers ONGOING a la carte pricing for all of its clinic services with no contract! While insurance plans and HSA/FSA providers do not reimburse Direct Primary Care membership fees in Texas, they do cover time of service (a la carte) fees. If you want to use your insurance, HSA, FSA or CareCredit to pay for BloomMed services, sign up for the A la Carte Program!
